For your convenience, the Seal Beach Police Department is pleased to make the following forms available online. It is the hope that in doing so the public will save time and ease the process of submitting various forms.
This document should be used to receive copies of traffic collision reports, incident reports, arrest reports, and information-only reports.
The oversized vehicle permit booklet allows a citizen to request six one-time use permits. Permits are valid for 24 hours and cannot be used consecutively. Please read all the rules and regulations found within the permit application.
Please complete all forms and return them to the Records located at the Police Department.

An alarm permit is required if you have an alarm system on any premises that directly or indirectly signals the Seal Beach Police Department.
